We routinely poll Cash-Based practice owners and ask what they think is holding their business back… what’s their “biggest problem/challenge”?.. And in all our surveys over the years, 60-75% of them answer “lead generation” / “I need the phone to ring more.”

But after more than 15 years of tracking the patient lifecycle across hundreds of practices, and identifying all the profit leaks and missed opportunities (lead gen, phone conversions, clinic conversions, pt retention, reactivation, etc), I’ve found that the numbers often tell a very different story about the most common and costly problem in cash practices…

The data shows that the largest, most common problem is that too many prospective patients never become paying patients… the phone is ringing enough for that practice to be booked out, but too few callers are being converted into customers…. 

We’ve found that improving phone conversions by just 10–30% usually fills the remaining gaps in your schedule without spending another dollar on marketing… and that level of improvement is not difficult with a proven phone call framework and scripts….

You May Not Need More Leads

I’ve looked closely at the entire patient lifecycle in hundreds of practices, from lead generation and phone conversions through retention, reactivation, referrals, and long-term patient value.

One pattern continues to show up: practice owners who aren’t tracking their phone conversion rates usually overestimate how well those calls are going. They see openings on the schedule and naturally assume they need more leads, when the math often shows they could fill those openings simply by converting more of the opportunities they already have.

Start With Two Columns

If you aren’t currently tracking prospective patient calls, please don’t turn this into another complicated project.

Open a spreadsheet and track two things: every legitimate prospective patient who calls and whether they convert into whatever the next step is for your practice. That’s it.

After a month, you’ll have something most practice owners don’t have: actual data showing your practice’s phone call conversion rates. You may discover the problem isn’t where you thought it was.

As they say, what gets measured gets managed.

What Should Your Conversion Rate Be?

Over the years, I’ve studied the psychology, sales research, scripting, and objection handling behind these calls while coaching practice owners and receptionists through thousands of conversations. That work eventually became my eight-step Cash-Pay Conversion System.

When the person answering your phones follows that system consistently and gets enough repetitions, they typically get to a point that they are converting roughly 75–85% of qualified prospective patients into paying customers.

A $68K Month With a Huge Profit Leak

One of my Mastermind members recently texted me after her rapidly growing practice had its best month ever: $68,000 in revenue, during summertime no less.

But her practice was also beta testing technology I’ve developed to automatically track these calls, and the initial data showed her two receptionists were converting only 38% of prospective patients.

Think about that. 

The practice just had its best month ever while converting just under 4 out of every 10 prospective patients. When we move that toward the 75–80% range I’ve seen repeatedly happen with my Cash-Pay Conversion System, the existing lead flow could potentially support well over $100,000 in monthly revenue, assuming the practice clinicians have the capacity.

That’s why conversion can be such powerful low-hanging fruit. Sometimes you don’t need more Google Ads, workshops, Facebook campaigns, or referral sources. You simply need to stop losing so many of the people you’re already attracting.

What Could 20% Better Conversion Be Worth?

Let’s use a smaller practice as an example. Imagine you receive ten legitimate prospective patient calls each week and currently convert 50%, or 5 out of 10.

Improving from 50% to 70% gives you two additional patients every week (remember, without generating one additional lead or spending one more dollar on marketing). Over roughly 50 working weeks, that’s 100 additional patients per year.

If your average plan of care is worth around $1,000, that’s approximately $100,000 in additional annual revenue before even accounting for returning patients,  word of mouth referrals, and their true lifetime value.

The Final Word

Before you spend another dollar trying to make your phone ring more, find out what’s happening when it already does.

Start tracking prospective patient calls and conversions, and keep it ridiculously simple at first. Once you know the number, you can determine whether you actually have a lead-generation problem or whether your biggest opportunity is converting more of the people already reaching out.

A relatively small improvement can be worth tens of thousands, and sometimes more than $100,000, in additional annual revenue. That’s why I believe improving phone conversions is one of the lowest-hanging opportunities available to most Cash-Based practices.
