Episode 300 feels like a good time to zoom out.

I started this podcast back in 2014. It was the first podcast on this topic of Cash-Based practice. Since then, I’ve had a front-row seat to startup clinics, multi-location operators, practice owners who were barely hanging on, and practice owners who were absolutely crushing it.

And when you watch enough practices long enough, patterns become impossible to ignore.

What’s Working Right Now

The biggest winners right now are not the practices doing the most marketing.

They’re the ones with the clearest positioning.

As Cash-Based competition grows, one-on-one care and “high-level treatment” are no longer enough to stand out by themselves. What stands out now is clarity around who you serve, what problem you solve, and why you are the obvious choice.

The second thing working is visibility. In a world flooded with AI-generated content, practices that still feel personal, real, and human have an edge.

And third, operational excellence is pulling practices apart faster than marketing ever did. Better response times. Better front desk training. Better follow-up. Better retention. Better continuity programs. Better KPI tracking. That’s where a lot of the real separation is happening.

What’s Quietly Failing

A lot of practice owners are overbuilding tech before they’ve built reliable patient-generation fundamentals.

Fancy funnels, automations, CRMs, and AI tools are getting too much attention too early. If you haven’t nailed your niche, your messaging, and your evergreen ways of getting patients in the door, you probably do not need more tech.

Old-school relationship building still works… often much faster than many forms of digital marketing.

Clinician Recruiting the way you always have is also quietly failing. Hiring is no longer something you can think about only when you need someone. You are always in the recruiting and retention business now… and if you don’t move forward with that as a priority, your practice will suffer in the long run.

And for many practices, the real thing quietly failing is leadership. Once you get beyond the startup phase, the next level usually doesn’t come from more tactics. It comes from better leadership, harder conversations, better delegation, better systems, and better accountability.

What’s Coming Next

The first major shift is AI discovery.

More people are asking AI tools for answers instead of clicking through search results. That means visibility is changing from “How do I rank on Google?” to “How do I get recommended by AI?”

The second shift is that the middle is getting squeezed. Insurance-Based practices will keep feeling pressure, and many will respond by offering more Cash-Pay services and modalities. That means more competition, more price shopping, and more pressure on practices that are vague or undifferentiated.

The third shift is AI and robotics. Anything that is mostly informational is becoming more vulnerable. The longer runway belongs to services that are harder to replicate digitally: hands-on care, manual therapy, procedures, modalities, and human judgment layered onto complexity.

The practice owners who adapt early will have a major advantage.
