If you’ve ever wondered if the cash-based model would work for an outpatient Neuro practice, wonder no longer! Re-Active PT and Wellness is a successful multi-therapist Neuro outpatient practice that is completely out of network with all payors except Medicare. Along with their skilled PT offerings, they also offer Occupational Therapy and a number of cash-pay wellness services. In this interview, owner Julie Hershberg PT, DPT, NCS tells us all about how she and her team have created this rewarding and inspiring private practice.

What a great episode! Many thanks, Julie and Jarod! I’ve been listening to the CBP and have found only a few neuro-based cash-based clinicians so it’s great to finally hear from one. I started a cash-based PT and wellness practice in Austin, TX centered around working with people with Parkinson’s Disease. Right now it is in-home to save on costs, etc. but I’d eventually like to construct a type of “destination wellness clinic” here in the Austin area that houses multiple practitioners and offers wellness services as well. It sounds like you started in an environment with quite a few resources for you to utilize, but do you have any recommendations or helpful resources for a solo-practitioner who is trying to get all of this off the ground?

Hi Sarah, Great to hear from another cash based neuro PT! We have had luck partnering with other organizations as well–such as Rock Steady Boxing and other local PD-specific groups. They have been a great resource for us and wonderful for cross-referrals. As we have more recently needed more space, we have partnered with a gym for people with neurologic disabilities, NextStep Fitness. They were interested in having a PT presence in their gym, so the relationship has been mutually beneficial. I am sure there are other similar resources in Austin. Good luck and keep in touch–it would be great to have a nationwide network of providers!
So glad to hear some neuro based info! WooHoo! I have started a home based neuro practice this past year and haven’t regretted it yet! My niche is specifically neuro trauma/ ABI and this lets me really concentrate on these complicated patients. Private care for neuromotor disorders is desperately needed! -Monika in Memphis, TN

I have been following your website and podcasts for quite some time now, and I would like to extend a huge thank you for the information that you and practitioners such as Dr. Hershberg provide.
As a first semester DPT student I was excited and interested to hear about the idea of student clinical rotations for pro bono services. I think that most clients paying cash for a high level of service would not be inclined to have a student practice on them, but in certain financial situations, a student/pro bono model would highly benefit both the student’s education and the practice! I think I need to mention this to my Director of Clin Ed!
